Assessor - AGING & DISABILITIES

Department:  Aging & Disabilities

Pay:  $17.71 per hour.  

Work Schedule: Monday - Friday 8:00am - 5:00pm

Administer annual functional assessments and comprehensive options counseling for assigned caseload of eligible Individuals with Developmental Disabilities (I/DD).

Comple annual functional assessments

• Schedule and coordinate meetings with the consumer, family/guardian, and service provider(s) to complete assessments annually by due date
• Travel to various locations in Sedgwick County to conduct assessments
• Collaborate with individuals and stakeholders to conduct thorough onsite or virtual assessments
• Review all documentation provided by stakeholders
• Use information from meeting to complete assessment and make eligibility determination
• Ensure all data is accurately collected, recorded and reported on the assessment

Provide annual comprehensive options counseling

• Provide annual options counseling during functional assessment meeting
• Document options counseling meeting including information on service(s), provider(s) and funding options

Minimum Qualifications: Six months of experience working with individuals with Intellectual / Developmental Disability (I/DD) -AND- bachelor’s degree from an accredited college or university. Education may be substituted with experience, with six (6) months’ experience working with I/DD for each semester.  Per Sedgwick County Policy, this is a driving level position that requires a valid US driver’s license without restrictions and proof of automobile insurance.  Must pass KBI, DCF child abuse, adult abuse registry, and motor vehicle screens.
