Certification in the State approved risk / needs assessment and case plan development. Collaboration with youth and families to ensure juveniles receive a risk and needs assessment, case plan and coordinate services while in detention; collect and maintain data; and manage the Individual Justice Plans (IJP) for Sedgwick County. Coordinate, communicate, document, and follow-up on discharge planning activities for youth, families and stakeholders.
Care Coordination
- Ensure detained juveniles receive a standardized risk and needs assessment within 72 hours of admission, or has appropriate updates made to such assessment if one has already been conducted
- Complete or update a case plan within 48 hours after the assessment has been conducted or updated
- Review case plan and coordinate recommended mental health and/or substance abuse assessments with contracted providers. If applicable, coordinate individual and/or group behavioral health, mental health, and substance use treatment disorder services with contracted providers for juveniles while in detention.
- Provide case file documentation of assessment, case plan, coordinated treatment, and outlined plan for discharge
- Collaborate with service providers to coordinate transitional services and discharge planning upon release from detention for continuity of care
- Communicate discharge plans with youth / families and stakeholders associated with the case.
- Follow-up on discharge planning activities and community treatment status for tracking purposes.
Individual Justice Plans (IJP)
- Collaborate with system stakeholders to facilitate multidisciplinary team (MDT) meetings to develop and maintain IJPs for Sedgwick County juveniles
- Facilitate stakeholder meetings/ trainings on IJP processes
- Collaborate with potential new community partners and update the MDT Order as necessary
- Serve as the IJP POC for the Sedgwick County Department of Corrections
Data Management
- Maintain records and data for the coordination of services and IJP process as deemed necessary
- Maintain and present a Coordination Report to the Detention Utilization Committee (DUC) monthly
Minimum Qualifications: Bachelor’s degree from an accredited college or university in Criminal Justice, Social Work, Counseling, Psychology or a closely related field. Transcript(s) required to verify education credits. Two years of experience in corrections or human services organizations working with youth and families. Professional references documenting the years of related work experience are required. Must be at least 21 years of age. Per Sedgwick County Policy, this is a driving level position that requires a valid US driver’s license without restrictions and current proof of automobile insurance. Personal vehicle for use when county vehicle is not available (employee reimbursement is provided by County). Certification in the State approved risk/ needs assessment and case plan development within initial period of employment. Pass legal background check, Child and Adult Abuse & Neglect Central Registry checks, and pre-employment screenings. Must also meet the required conditions contained in KSA 65-516 and KAR 28-4-356.
