Work Schedule: Varied schedule Monday-Saturday, 7:00am -7:00pm, up to 19 hours per week
Provide goal-directed, patient-centered, medically necessary services and support to children with Serious Emotional Disturbance and their families in order to improve the quality of life and ability to function more successfully in the community. Computer documentation of all services in a timely manner is required.
Psychosocial Rehabilitation- Individual and Groups and Attendant Care
- Facilitate Psychosocial Group and Individual treatment, that are self-contained, goal-directed groups designed to assist patients in minimizing or resolving the effects for mental and emotional impairments
- Objectives of the group activities shall be designed to: assist individuals with daily problem solving; improve social skills; provide leisure time training; promote health and enhance personal relationships
- Provide mental health Attendant Care (AC), one-to-one support or supervision with the goal of maintaining the patient in natural community locations, such as at home, in school or in the community
- Direct support and supervision in activities of daily living or to support the patient and/or the family in maintaining daily routines critical to a stable lifestyle
- Provide transportation in personal passenger vehicle to support patient attendance in medical appointments and other therapeutic goal-related activities to include development of community resources, employment or education access, social or consumer-run activities and related supports
Treatment Planning and Care Coordination
- Participate in development and completion of a Treatment Plan including patient, family, and/or legal guardian participation on cases where needed
- Identify, arrange, advocate, and collaborate with other agencies for treatment options and services provided that address patients' individual needs
- Expanding relationships with other agencies through the development of memorandums of understanding, sharing relevant health information, developing referral networks, and utilizing community resources to their fullest extent
Documentation
- Complete progress notes in accordance with COMCARE and CCBHC guidelines which meet requirements for medical necessity, goal-directed treatment and patient response to treatment within prescribed timeliness
- Complete Notes for Record, Precautions statements and authorizations for release of records as required and/or recommended
- Enters no show and cancellations for accuracy and completeness of medical record
Minimum Qualifications: Bachelor’s degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education. Per Sedgwick County policy, this is a driving level position that requires a valid US driver’s license without restrictions and current proof of automobile insurance. Must have access to a personal passenger vehicle to complete the driving duties of the position. Meet the specifications as outlined in the CMHC/CCBHC and/or CCBHC licensing standards and pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens. Must complete orientations provided by Sedgwick County and COMCARE.
Preferred Qualifications: Demonstrated ability to work with preschool age youth and their families and knowledge of their unique needs.
