Collaborate with KDOC system to ensure the client that are involved with the KDOC system receives the best possible community based services.  Coordinates services for individuals releasing from prison into the community. Ensures that all patients that are released from a prison facility engage fully in outpatient services post release by engaging with consumers frequently during the transition to the community.

 

Direct Client Service

  • Create plans to ensure clients get to their mental health appointments
  • Confer with client caregivers, families, physicians, lawyers, community service providers and other professionals throughout the County and to secure and coordinate services
  • Assist clients to maintain basic needs (housing, food, medical)
  • Provide outreach, screening and engagement services to potential participants
  • Respond to the immediate needs of referrals
  • Arrange for the discharge of inpatient referrals and coordination of transition to the community
  • Provide outreach services to reengage consumers who have been recently released from prison.
  • Outreach individuals currently on parole.
  • Provide transportation to support client attendance in medical appointments and other therapeutic goal related activities to include development of community resources, employment or education access, social or consumer-run activities and related supports.

 

Facilitate Discharges from KDOC facility

  • Assist in implementing release plans for clients in the KDOC system.
  • Provide frequent contact with clients for 3-4 weeks post discharge from the facility.
  • Research and coordinate un-tapped services in the community and state for particularly difficult patients to serve.
  • Prepare written discharge plans to COMCARE staff, affiliate agencies and KDOC staff prior to a client being discharged.
  • Monitor the implementation of release plans to ensure the clients have all recommended services promptly on their discharge from the prison.
  • Act as a liaison between all the COMCARE programs and affiliates regarding consumers that are currently incarcerated. Provide feedback and direction to medical team regarding the determination of services needed, availability, barriers and solutions.
  • Work to remove service barriers to enhance service delivery upon release from facility to prevent further legal charges.

 

Treatment Planning and Documentation

  • Prepare Treatment Plan including client and family participation on cases where needed.
  • Collaborate in review, updates and progress on goals with Treatment Plan coordinator or other members of the treatment team.
  • Complete progress notes in accordance with COMCARE and CCBHC guidelines which meet requirements for medical necessity, goal-directed treatment and client response to treatment within prescribed timelines.

 

Minimum Qualifications: Bachelor’s degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education. If no degree is present applicant may substitute each one year of experience for one year of education. Must have a valid Kansas Driver’s license and current proof of automobile insurance. Must be able to safely operate a motor vehicle and meet driver qualification standards as set forth in Sedgwick County’s Fleet Vehicle Operation and Usage policy.  Meet the specifications as outlined in the CMHC/CCBHC licensing standards and pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens.  Must complete orientations provided by Sedgwick County and COMCARE.
