Work Schedule: 40 hours per week, Flexible schedule Monday - Saturday 7:00 am - 7:00 pm

The Certified Community Behavioral Health Clinic (CCBHC) Assertive Community Treatment (ACT) Integrated Care Specialist will provide goal-directed, patient-centered, medically necessary services and support to adults with severe and persistent mental illness exhibiting an increase in symptoms decreasing their ability to manage safely in the community. Specialist will be available to provide one on one service in the home and/or community to the patient as well as assisting with coordination of care through treatment planning and referrals. The specialist will be working with shortened timelines for task completion to ensure patient’s needs are met quickly with the goal of stabilization of acute symptoms. Computer documentation of all services in a timely manner is required.

 

A CCBHC employee must be able to complete complex tasks by applying CCBHC practices, standard office policies, authorized instructions, and past precedents to achieve a desired outcome. Employee must quickly recognize emergency or sensitive situations and take appropriate action. Critical decision making and the ability to think through the consequences of a decision are essential in this position.

 

Service Provision Case Management Services

  • Case management using identified and accepted methods to remediate symptoms of mental illness and/or improve emotional and functional well-being while meeting the expectations of the CCBHC ACT model.
  • Conduct risk assessments as needed to address safety of patients
  • Provides transportation to support patient attendance in medical appointments and other therapeutic goal related activities to include development of community resources, employment or education access, social or consumer-run activities and related supports

 

Treatment Planning

  • Prepare Treatment Plan including patient and family participation on cases where needed
  • Collaborate in review, updates and progress on goals with Treatment Plan coordinator or other members of the treatment team
  • Meet with patient to complete strengths assessment, discuss treatment goals and available services

 

Documentation

  • Complete progress notes in accordance with COMCARE and CCBHC guidelines which meet requirements for medical necessity, goal-directed treatment and patient response to treatment within prescribed timeliness
  • Complete Notes for Record, Precautions statements and authorizations for release of records as required and/or recommended
  • Enters no show and cancellations for accuracy and completeness of medical record
  • Enter AIMS data as needed

 

Minimum Qualifications Bachelor’s degree in a human services field, or an equivalent combination of education and human services experience, with one year of experience substituting for one year of education. Per Sedgwick County policy, this is a driving level position that requires a valid US driver’s license without restrictions and current proof of automobile insurance. Must have personal passenger vehicle for transportation of materials/individuals for essential functions.  Meet the specifications as outlined in the CMHC/CCBHC licensing standards and pass KBI, DCF child abuse check, DCF adult abuse registry, KDADS Nurse Aide and Criminal Record check, National Sex Offender Registry check, and motor vehicle screens.

 

Preferred Qualifications:  One year working in a Community Mental Health Center or Certified Community Behavioral Health Clinic type setting.
