Support the provision of goal-directed, medically necessary services and support to foster care involved youth with severe emotional disturbances, their families, and Certified Community Behavioral Health Clinic (CCBHC) patients. Assist families and treatment teams to maintain children in the least restrictive/least intrusive environment. Advocate for children and families as needed.

 

Employee must be able to complete complex tasks by applying CCBHC practices, standard office policies, authorized instructions, and past precedents to achieve a desired outcome. Employee must quickly recognize emergency or sensitive situations and take appropriate action. Critical decision-making and the ability to think through the consequences of a decision are essential in this position.

 

Systems Liaison

 

  • Serve as COMCARE's designated system liaison for Child In Need of Care (CINC) youth with Managed Care Organizations, Department of Children and Families (DCF), Kansas Department of Aging and Disability Services (KDADS), foster care agencies, courts, and community providers.
  • Coordinate communication regarding admissions, discharges, transitions, and diversion planning as needed.
  • Ensure designated CINC phone and email communication and respond within required contractual timeframes.
  • Coordinate transition of care when youth relocate outside COMCARE's catchment area.
  • Facilitate information sharing consistently with applicable confidentiality requirements and Memorandums of Understanding.

 

Clinical Coordination and Continuity of Care

 

  • Monitor foster care youth admitted to acute psychiatric and Psychiatric Residential Treatment Facilities to facilitate continuity of care, identify barriers to treatment, coordinate communication among providers, and support timely transition planning.
  • Provide follow-up after a reported mental health crisis to ensure appropriate levels of services are provided, and adjustments are made based on medical necessity.
  • Participate in multidisciplinary treatment team meetings to promote collaboration among internal clinical teams, community providers, child welfare partners, and legal guardians while supporting the least restrictive treatment environment as needed.

 

Contract Compliance, Reporting, and Quality Monitoring

 

  • Complete and maintain required CINC Workbook reporting.
  • Collect and verify required CINC data elements.
  • Submit required quarterly reports within established deadlines.
  • Monitor contractual access standards
  • Coordinate submission of required clinical documentation to foster care agencies within contractual timeframes.
  • Track system barriers that impact service access and work with leadership to resolve barriers.
  • Assist with compliance reviews and audits.
  • Maintain documentation in accordance with KDADS, CCBHC, DCF, and COMCARE requirements.

 

Minimum Qualifications: Bachelor’s degree or equivalently qualified by work experience or a combination of work experience in a human services field and education, with one year of experience substituting for one year of education. One (1) year of experience in a Community Mental Health Center setting. Per Sedgwick County policy, this is a driving level position that requires a valid Kansas driver’s license without restrictions and current proof of automobile insurance. Meet the specifications as outlined in the CMHC/CCBHC licensing standards and pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens. Must complete orientations provided by Sedgwick County and COMCARE.
