Care Resource Team Leader COVD-19

This is a temporary position in the Sedgwick County Health Department for COVID-19 response. Grant funding for this position is through a grant that ends November 30, 2022. The Care Resource Team Leader must be bilingual in English and a language other than English. The position supervises part-time Care Resource Coordinators. The Care Resource Team improves health access by linking clients to community resources, assisting clients in identifying services available to meet their needs. The Team Leader ensures the team communicates in a culturally and linguistically appropriate manner and is viewed as a trusted resource to the communities served. The Team Leader ensures patient confidentiality and may wear personal protective equipment (PPE) during the course of their work.


Coordinate and supervise Care Resource Coordinators

  • Plan and coordinate staffs’ part-time schedules, ensuring client needs are met
  • Supervise and evaluate the performance of staff through observations, audits, and team meetings
  • Continually monitor compliance with protocols
  • Drive to SCHD locations to supervise and provide services


Ensure the Care Resource Coordination Team provides quality client services

  • Write protocols and procedures; ensure they are up-to-date and followed
  • Ensure daily follow-up and ongoing informal counseling and social support is provided to clients
  • Document activities and complete standard reports for the Care Resource Team and grant requirements


Provide leadership for care coordination among communities and internal and external community partners 

  • Develop and implement studies with partners to increase access to testing and reduce the COVID-19 case rate among the communities served
  • Analyze COVID-19 testing and case data of the communities served to determine impact of educational interventions
  • Organize communities, County staff, and external partners to identify and find ways to fill critical service gaps


Minimum Qualifications: High school diploma or GED. One year of experience working or living in a multicultural setting.  Four years of communications or customer service in a health or medical field, providing referrals to resources, and/or coordinating with community partners. Work experience supervising staff such as scheduling, auditing or evaluating performance. Proficiency in English and a language other than English. Proficiency is demonstrated by passing a standardized test.  Per Sedgwick County policy, this is a driving level position that requires a valid Kansas driver’s license without restrictions and current proof of automobile insurance. In order to comply with the requirements of the federal health care staff COVID-19 vaccination mandate, all employees of COMCARE and the Health Department must be fully vaccinated. All offers of employment are contingent upon submission of vaccination documentation prior to start date. 


Preferred Qualifications: Bachelor’s degree in a health-, business- or communications-related field. One year health education experience. One year experience working as a community health worker or health navigator. Two years of experience working or living in a multicultural setting.  One year of work experience in a public health department or healthcare setting. One year of supervisory work experience, including employee evaluations. Completion of Community Health Worker training.
