Case Manager III - Crisis Services High Utilizer Case Manager - COMCARE

Pay:  $15.40 per hour | This position is currently eligible for a $2,500 sign-on bonus after 60 days of employment! | This position has also been approved for a salary increase effective January 1, 2022. New rate will be $19.64 per hour. 

Work Schedule: Varied Hours M-F 7a-7p(Some weekends required) 

Will maintain a caseload of individuals that are high utilizers of emergency services. Case manager will provide case manager services to individuals that are currently involved with KDOC or being released from inpatient psychiatric hospitalization. They will be able to maintain a schedule while also providing services for crisis related needs.


Direct Client Service
• Provide crisis stabilization services to individuals experiencing a mental health crisis while they are in the Crisis Residential Units
• Manage distraught clients who are in need of structure; monitor gross psychological and physical functions of clients
• Familiarize clients with guidelines and procedures of the residential units
• Identify and secure in-home services to maintain stabilization once client returns home
• Contact service providers to request and arrange services; coordinate and monitor services rendered
• Confer with client caregivers, families, physicians, lawyers, community service providers and other professionals throughout the County and to secure and coordinate services.
• Provide outreach, screening and engagement services to potential participants
• Respond to the immediate needs of referrals
• Provide outreach services to reengage consumers who have relapsed or are nonresponsive to treatment.
• Provide Targeted Case Management (TCM) for assigned consumers to aid in achieving goals identified in the individualized treatment plan. Treatment planning, collateral contacts, phone support, access to supports and service coordination.
• Provide Community Psychiatric Supportive Treatment (CPST) for assigned consumers to achieve treatment goals, assistance in improving symptom self-management, strengths assessment and planning, assistance to assure consumer income maintenance, individual therapeutic intervention, medication education, crisis management, assistance in to achieve academic progress.
• Provide Attendant Care (AC) for assigned consumers to achieve treatment goals. AC interventions include: direct support, supervision and/or cueing.
• Provide transportation to support client attending medical appointments and other therapeutic goal related activities.


Treatment Planning
Prepare quality treatment plans in collaboration with client or guardian. Facilitate treatment plan review, addressing updates and progress on goals with client, coordinator.
• Complete DLA-20 at initial case management appointment and quarterly thereafter.

• Complete Notes for Record, releases of information, safety documents and other documentation as required and/or recommended within established deadlines.
• Enters no show and cancellations for accuracy and completeness of medical record.
• Respond to Utilization Review staff feedback forms and make corrections as needed.
• Enter AIMS data monthly.
• Completes discharge summary when closing to case management services.
• Completes court reports or treatment summaries within required timelines as set by the entity requesting these documents.
• Ensure client’s financial information is current and up-to-date in medical record.


Minimum Qualifications: Bachelor’s Degree or equivalent education and experience. If no degree is present applicant may substitute each one year of experience for one year of education. Must have a valid Kansas Driver’s license and current proof of automobile insurance. Meet the specifications as outlined in the CMHC licensing standards and pass KBI, SRS child abuse check, adult abuse registry, and motor vehicle screens. Must complete orientations provided by Sedgwick County and COMCARE


Preferred Qualifications: 1-2 years of experience in a Community Mental Health Center and/or supervisory experience.
