Case Manager III - Early Childhood Specialist - COMCARE

Work Schedule: Varied schedule M-F between 7a -7p 

 

Provides goal-directed and client-centered services and/or interventions to early childhood clients, aged 3-6, who have been diagnosed with a Serious Emotional Disturbance. Services and interventions teach social and emotional regulation skills within both an early childhood classroom setting on-site at COMCARE Children’s and in the community to improve quality of life and ability for children to function in daycares, schools, home, and in the community at large. Computer documentation of all services in a timely manner is required.

 

Service Provision Case Management Services

  • Case management using identified and accepted methods to remediate symptoms of mental illness and/or improve emotional and functional well-being
  • Conduct risk assessments as needed to address safety of clients

Treatment Planning

  • Prepare Treatment Plan including client and family participation on cases where needed
  • Collaborate in review, updates and progress on goals with Treatment Plan coordinator or other member of the treatment team

Documentation

  • Complete progress notes in accordance with COMCARE guidelines which meet requirements for medical necessity, goal-directed treatment and client response to treatment within prescribed timeliness
  • Complete Notes for Record, Precautions statements and authorizations for release of records as required and/or recommended.
  • Enters no show and cancellations for accuracy and completeness of medical record.

 

Minimum Qualifications: Bachelor’s degree. If no degree is present applicant may substitute each one year of experience for one year of education. Must have a valid Kansas Driver’s license and current proof of automobile insurance.

 

Preferred Qualifications: One year of experience.
