Case Manager IV ERC Resource Specialist

Provides goal-directed, client-centered case management services for clients of the Evening Reporting Center (ERC) in an effort to reduce recidivism and increase successful functioning in the community. Case management services include assisting clients in understanding community systems, providing advocacy, provide client and family support and guidance, and maintaining statistical data regarding services provided to clients and families in the community. Employee must quickly recognize emergency or sensitive situations and take appropriate action. Critical decision making and the ability to think through the consequences of a decision are essential in this position.


Primary Job Functions:

  • 50% Case Management- . Conducts in-person interviews with clients, families, and caregivers to complete service plans. Secures available staff and community resources to meet client needs. Provides a consistent point of contact to clients for information and referrals to access health care, employment opportunities, housing, and any other necessary community resources. Acts as a liaison and assists clients in contacting and utilizing those resources and their services. Monitors client’s participation in service plan activities by making contacts with providers, employers, family members, and other professionals as appropriate. Encourages clients to develop interests which increase their productivity in the community and reduce their risk for recidivism. Maintains awareness of referral sources and community resources/service providers. Travel to client and families’ homes as required.
  • 30% - Program Services– Participates in individual and group contacts with youth. Provides crisis intervention services as needed. Provides program support in times of high client population and/or staff shortages. Serves as an in-house trainer on community resources. Maintains safety and security for clients and staff. May temporarily assume supervisory duties in the absence of a supervisor. Conducts and facilitates behavioral cognitive groups as assigned. Participates in multidisciplinary or peer team meetings to improve service provision and develop intervention strategies as needed.
  • 15% - Data Management – Assists with statistics gathering, tracking, and monthly/quarterly reports for the ERC Program. Accurately documents all client interactions in the appropriate databases within prescribed timeframes.


Minimum Qualifications: Bachelor’s degree in Sociology, Psychology, Criminal Justice, Social Work, or related field from an accredited college or university. Transcript verifying education credits is required. One year of experience working with juveniles. Facility staff shall be at lease twenty-one (21) years of age. Must possess first aid, CPR and physical restraint certification or the ability to acquire within initial period of employment.


Preferred Qualifications: Master’s degree in Sociology, Psychology, Criminal Justice, Social Work, or related field. Previous case management experience strongly preferred. Current working knowledge of community resources preferred.
