Center City Field Case Manager (Homeless Program) - COMCARE

Case Managers provide goal-directed, medically necessary case management services and support to clients with mental health needs. Because we care so much for the Clients we serve, we’re looking for reliable individuals who can commit to regular and non-disruptive work attendance.

Collaboration & Cooperation – by showing an interest and acknowledging Clients perspectives and ideas; and building relationships through listening and emphasizing to collaborate and find solutions.

Communication – having the ability to gently hold firm to ensure compliance with policies and laws; and having timely and current communication to consumers and treatment team members.

Detail Oriented - Excellent organizational skills, with the ability to set priorities and produce accurate and complete work

Time Management – having the ability to work at a higher level, be it quality or speed without having a significant impact on performance; is motivated to achieve and ensures that tasks are completed on time

As a Case Manager you will:

  • Provide Case management to individuals using identified and accepted methods to remediate symptoms of mental illness and/or improve emotional and functional well-being.
  • Conduct risk assessments to address the safety of our clients
  • Prepare Treatment Plans including the client and his/her family participation
  • Collaborate in review, updates and progress on your Client’s goals with the Treatment Plan coordinator or other members of the treatment team
  • Meet with your clients to complete strengths assessment, discuss treatment goals and available services
  • Complete progress notes which meet the requirements for medical necessity, goal-directed treatment and client response to treatment within prescribed timelines.

The current openings fall under the following program:

  • Center City case managers work with homeless individuals who have also been diagnosed with a mental illness.

Applicants must possess a Bachelor’s degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education. Must have a valid Kansas Driver’s license and current proof of automobile insurance, and pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens.
