Community Health Nurse II-HEALTH

Provide direct in-home prenatal and parenting education and support to women and families in Sedgwick County with a focus on the reduction of infant mortality, low birth weight and prematurity among high-risk mothers. This education and support is provided in both the home and the doctor’s office as providers will accompany clients to well-child visits and assist with navigation through the healthcare system.


Home visits/In-office visits

  • Complete in-home/ doctor’s office visits following program guidelines
  • Evaluate clients for mental and physical health barriers by performing a variety of assessments.
  • Ensure a Welcome Home visit is completed once client has delivered.



  • Documentation will occur in real time at client visits, with full completion in 48 hours.
  • Audits of documentation will be revised and returned to coordinator by deadline given.



  • Provide maternal child health education
  • Conduct developmental screenings per curriculum timeframe
  • Complete referrals to other community agencies based on individual client needs.
  • Participates and represents Healthy Babies in a professional manner by conducting presentations at health fairs, community events, and coalitions and for other community agencies.


Minimum Qualifications: Associate’s Degree in Nursing. One year experience in public health, labor and delivery, postpartum or pediatric nursing. Current Registered Nurse license in the state of Kansas required. Attends required trainings to maintain licensure and/or certification. In order to comply with the requirements of the federal health care staff COVID-19 vaccination mandate, all employees of COMCARE and the Health Department must be fully vaccinated. All offers of employment are contingent upon submission of vaccination documentation prior to start date.


Preferred Qualifications: Bachelor’s Degree in Nursing (BSN) preferred. One year experience post BSN. Home visiting experience preferred. A minimum of one year working in a hospital setting preferred. Bilingual English/Spanish skills preferred. Knowledge in early childhood development and prenatal education preferred. Current CPR certification preferred.
