The Community Health Worker Overdose Prevention works in different organizations and businesses in Sedgwick County to connect people at risk of opioid or stimulant overdose to community services. In this way, the position prevents disease and promotes wellness. The position provides case management, building individual client health knowledge and community capacity to serve the community. The position is a trusted resource in the community and must provide tailored communications to clients and accurately document client work in a database. Employee will provide support during public health emergencies.

Identify clients at risk of overdose in need of referral to services

• Partner with supervisors, SCHD staff, and community organizations to identify at-risk populations and implement client identification, welcoming, assessment, and referral strategies.
• Research, identify, and maintain community resources that help clients overcome barriers and achieve health goals.
• Meet with referral sources and community organizations to confirm service availability and compatibility.
• Coordinate with medical and non-medical facilities, outreach teams, and community partners.
• Travel to client locations, including hospitals, clinics, shelters, and other community settings.
• Serve as a trusted community resource through formal and informal education, presentations, and indoor/outdoor outreach activities.

Provide community referral information and health education to clients

• Provide individual case management, health education, and support to help clients set and achieve goals.
• Communicate with clients to identify goals, provide tailored information, and connect them with appropriate services.
• Make referrals to community agencies in person, by telephone, or through referral platforms and systems.
• Assist clients with scheduling appointments, as appropriate.
• Follow up on referrals to assess status and determine whether additional assistance is needed.

Administrative Operations

• Provide referral-related technical assistance to SCHD staff and community partners.
• Assist with needs assessments, surveys, data collection, outreach event planning, and partner meetings.
• Enter and maintain accurate client information and goals in the database; complete self-audits.
• Identify and recommend improvements to referral procedures and workflows.
• Assist with evaluating referral system effectiveness.
• Maintain subject-matter expertise through training and research of available resources.
• Provide support during public health emergencies.

Minimum Qualifications: High school diploma or equivalent. One year of work experience using word processing, spreadsheets, and email applications such as in Microsoft Office. One year of customer service work experience. One year of data entry experience. Per Sedgwick County Policy, this is a driving level position that requires a valid US driver’s license without restrictions and current proof of automobile insurance. Must complete Federal Emergency Management Administration (FEMA) IS 100 and 700 courses within 30 days of employment. Obtain Community Health Worker certificate within 1 year of hire while following training plan during that time.

Preferred Qualifications: Associate’s degree in business or a health-related field. One year of health education experience. One year of experience working as a community health worker or health navigator. One year of work experience in a public health department or healthcare setting. One year of experience with referring clients to community resources.  One year of experience documenting work with clients. Community Health Worker certificate.  Proficiency in English and a language other than English; proficiency is demonstrated by passing a standardized test.
