Healthy Babies Community Liaison - Health

Provide direct in home prenatal and parenting education and support to women and families in Sedgwick County with a focus on the reduction of infant mortality, low birth weight and prematurity among high-risk mothers. Education and support is both provided at in home and doctors office visits as provider will accompany clients to Well child visits assisting with naviation in the healthcare system. This position works to meet the mission of the Sedgwick County Health Department by promoting the wellness of Sedgwick County residents through the Healthy Babies Program’s focus on reduction of infant mortality, low birth weight and prematurity among high risk moms.

 

Home visits/In-office visits

  • Complete in-home/ doctor’s office visits following program guidelines
  • Evaluate clients for mental and physical health barriers by performing a variety of assessments.
  • Ensure a Welcome Home visit is coordinated with a HB RN once client has delivered.

 

Documentation

  • Documentation will occur in real time at client visits, with full completion in 48 hours.
  • Audits of documentation will be revised and returned to coordinator by deadline given.

 

Education

  • Provide maternal child health education
  • Conduct developmental sceenings per curriculum timeframe
  • Complete referrals to other community agencies based on individual client needs.
  • Participates and represents Healthy Babies in a professional manner by conducting presentations at health fairs, community events, coalitions and for other community agencies.

 

Minimum Qualifications: Bachelor’s degree in Social Work/Public Health or related field. Minimum 1 year field case management, home visiting experience with multi-stressed families, or any combination of training and experience which provides the required knowledge, skills, abilities and competencies. Per Sedgwick County policy, this is a driving level position that requires a valid Kansas driver’s license without restrictions and current proof of automobile insurance. In order to comply with the requirements of the federal health care staff COVID-19 vaccination mandate, all employees of COMCARE and the Health Department must be fully vaccinated. All offers of employment are contingent upon submission of vaccination documentation prior to start date.

 

Preferred Qualifications: Minimum 5 years field case management, home visiting experience with multi-stressed families, or any combination of training and experience which provides the required knowledge, skills, abilities and competencies. Social Work licensure by BSRB . Bilingual preferred.
