The Healthy Babies Healthy Start and MCH Coordinator provides day-to-day supervision of Home Visitors and a Maternal and Child Health Community Health Worker under three grants. Provides administrative support to the Healthy Babies Program Manager and participates in program planning, personnel management, team meetings, and procedural improvements. This position supports the Sedgwick County Health Department’s mission by promoting wellness through the Healthy Babies Program’s focus on reducing infant mortality, low birth weight, and prematurity among high-risk moms. Employee will support public health emergencies (Activation Level 1).
Coordinate daily Home Visitor and Community Health Worker activities.
- Manage and evaluate staff outcomes and expectations related to grant requirements.
- Review weekly and monthly reports on Home Visitor and CHW compliance with grant outcomes.
- Work with the Program Manager and Senior Admin. Officer to ensure program outcomes are met.
- Meet with Home Visitors and CHW weekly to monthly to discuss progress toward outcomes.
Compile monthly data reports to ensure timely, accurate completion and submission of required program documents.
- Monitor and manage Home Visitor and CHW caseloads for equitable distribution and expected caseload levels.
- Review weekly and monthly reports and provide feedback to the Healthy Babies Program Manager as requested.
- Monitor home visitors’ and CHW progress towards achieving program requirements as stated in program grants. This position supervises home visitors and CHW under three different grants.
- Use tools such as data tracking spreadsheets to track home visitor and CHW progress.
- Set clear deadlines for Home Visitor and CHW assigned tasks.
- Review electronic charts during monthly supervision to ensure accuracy, completeness, and program outcomes are met.
- Complete quality-assurance audits of client health records and Home Visitor and CHW documentation.
Participate in and facilitate building and maintaining internal and external partnerships.
- Represent Healthy Babies in a positive manner and promote the program goals and mission to foster positive support from community partners.
- Attend and participate in community coalitions as requested.
- Provide Healthy Babies program objectives and participant criteria to community partners as requested.
- Promote provider participation by attending at least three community outreach events, in addition to Healthy Babies Community Baby Showers.
- Lead and/or assist with agency quality-improvement processes.
- For Healthy Start clients, partner with the Fatherhood Coordinator to engage fathers in HS services.
Minimum Qualifications: Bachelor’s Degree from an accredited college or university in social work, health science, psychology, sociology, counseling, or a similar community service-oriented field. One (1) year of experience in a social service or case management position. One (1) year of supervisory experience. One (1) year of experience with data management. Experience with public speaking and presentation development. Per Sedgwick County Policy, this is a driving-level position requiring a valid, unrestricted Kansas driver’s license and current proof of automobile insurance. For Activation Level 1 staff: Must complete Federal Emergency Management Administration (FEMA) IS 100 and 700 courses within 30 days of employment. Additional training may be required.
Preferred Qualifications: Two (2) years of experience in social service or case management. Two (2) years of supervisory experience or equivalent combination of training and experience providing the required knowledge, skills, and abilities. Experience working in a grant-funded position. Current Kansas BSRB license.
