Pay:  $24.63 per hour | This position may also be eligible for a sign on bonus of $2,500 after 60 days of employment if selected applicant meets all policy conditions. | This position may be eligible for shift pay of $3.00 per hour for hours worked between 6:00 pm and 6:00 am.  

Work Schedule: Monday - Friday 8:00am - 4:30pm

Collaborate with KDOC system to ensure the patients accepted into the Kansas Supportive Housing for Offenders Program (KSHOP) receives the best possible community-based services. Coordinates services for individuals releasing from prison into the community and provide intensive case management to engage consumers, assist with accessing community resources to address basic needs and to identify long term solutions in an effort to promote recovery.

 

Service Provision Case Management Services

  • Work with KDOC and parole staff to address system issues that create barriers to offenders receiving mental health care timely and effectively, engaging in strategic planning and implementation of identified solutions.
  • Participate in case staffing with staff in the correctional facilities, parole, along with other treatment providers, peer mentors, or other persons working with the offender on reintegration, risk reduction and addressing risk/needs, including treatment needs, providing input into case planning, and ensuring all services available through COMCARE for which the offender is qualified are available to that offender.
  • Assist KDOC and parole staff in completing applications for benefits, including disability, to assist in securing those benefits; support efforts to access other resources in the community for which the offender is eligible and would benefit the offender in reintegration and risk reduction.
  • Work with KDOC and parole staff, including the Mentoring Specialist, to identify peer-mentoring needs of offenders with mental illness, and assist KDOC and parole staff in connecting offenders to peer mentoring resources when available and as appropriate.
  • Assist in implementing release plans for clients in the KDOC system. 
  • Provide Community Psychiatric Supportive Treatment (CPST) for assigned consumers to achieve treatment goals; interventions include: assistance in improving symptom self-management, strengths assessment and planning, assistance to assure consumer income maintenance, individual therapeutic intervention, medication education, crisis management, assistance in to achieve academic progress
  • Provide transportation to support patient attendance in medical appointments and other therapeutic goal related activities to include development of community resources, employment or education access, social or consumer-run activities and related supports

 

Treatment Planning

  • Prepare quality treatment plans in collaboration with patient and/or guardian, additional treatment team and community partners
  • Complete DLA-20 at initial case management appointment and quarterly thereafter

 

Documentation

  • Complete progress notes in accordance with COMCARE and CCBHC guidelines which meet requirements for medical necessity, goal-directed treatment and patient response to treatment within set timelines
  • Complete discharge summary when closing to case management services

 

Minimum Qualifications: Bachelor’s degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education. Per Sedgwick County policy, this is a driving level position that requires a valid US driver’s license without restrictions and current proof of automobile insurance. Must have personal passenger vehicle for transportation of materials/individuals for essential functions. Meet the specifications as outlined in the CMHC/CCBHC licensing standards and pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens. Must pass KDOC background check to enter correctional facilities and parole offices to perform duties of the position. Must complete orientations provided by Sedgwick County and COMCARE.
