Medicaid HCBS Program Coordinator-AGING

Provide oversight and direction, critical program decision making and staff supervision for the Medicaid Home and Community Based Services (HCBS) Waiver program. Performs supervisory duties, functions and act as back-up for Eligibility and Options Specialist assessor(s) and FAI scheduling staff when needed. Provides input on hiring decisions. Performs liaison activities with Kansas Department for Aging and Disability Services (KDADS), Kansas Department for Health and Environment (KDHE) and KanCare Managed Care Organizations (MCO’s) to ensure compliance with state and federal laws, regulations, program policies and the financial results of the program. Point of contact for the Department regarding the assignment, completion and quality review of Functional Assessment Instrument (FAI) assessments for the HCBS Brain Injury (BI), Physically Disabled (PD), Frail Elderly (FE) and Program for All- Inclusive Care for the Elderly (PACE) programs.

 

PROGRAM MANAGEMENT

  • Participate in hiring, staff orientation, and evaluates Eligibility and Options Specialist (EOS) staff competency and performance. Provides staff training, coaching, counseling and discipline as warranted. Provides Unit Director assistance in oversight of the Administrative Case Management (ACM) program as needed.
  • Maintain oversight of FAI staff scheduler to evaluate and improve the efficiency of scheduling processes to enhance quantity, quality, efficiency, and output.
  • Directs data entry function to assure efficiencies of assessment entry into MMIS Systems to insure Medicaid reimbursement.
  • Point of contact for HCBS Waiver programs for consumer’s, residential facilities, hospitals, family, KDADS, KDHE, MCOs and other professionals providing technical assistance, information on regulations, review and resolution of problems that may arise.

 

PERFORMANCE MANAGEMENT

  • Supervise program staff and maintain oversight of service provided, program revenue, staff activities, quality assurance of work, and data collection.
  • Provide one-on-one or group staff training on updated processes and program changes.
  • Provide technical assistance (TA) and program resolution to consumers, family support members, community stakeholders and other professionals.

 

ASSESSMENT & UNIT SUPPORT

  • Complete Functional Assessment Instrument (FAI) assessments for determination of HCBS eligibility including FE, PD, BI, PACE Medicaid assessments. Provide Options Counseling to all consumers and referral to Administrative Case Management (ACM) services.
  • Oversee the scheduling of staff, manage leave requests, approves timesheets, mileage, and maintains adequate coverage to ensure effective and timely delivery of services.

 

Minimum Qualifications: Bachelor’s degree from an accredited college or university in Social Work, Gerontology, Nursing, or Counseling. Three (3) years’ experience in the field of aging. Two (2) years’ supervisory experience including planning, work assignment and quality review, staff performance evaluation, and participation in the hiring process. Valid State of Kansas driver’s license and proof of automobile insurance.

 

Preferred Qualifications: Master’s degree from an accredited college or university in Social Work, Gerontology, Nursing, or Counseling. Four (4) years’ or greater of work experience in the aging field for an Area Agency on Aging with service to vulnerable populations: frail elderly, physically disabled, and traumatic brain injury, pre-nursing home assessment, and targeted case management experience (TCM). Three (3) years’ supervisory experience. Licensed Social Worker. AIRS, Information and Referral Certification. Bilingual skills a plus.
