This position assists eligible adults 60+ in accessing nutritious meals through congregate or home delivered meal programs. The position will be responsible for completing a person-centered assessment to review nutrition risk, special diet needs, access to nutritious meals, and evaluate eligibility.  Additionally, this position will provide part time support as a congregate site manager at designated meal locations as needed for the daily meal program along with scheduled nutrition education. This position will need to communicate proficiently via telephone, email and in-person with potential clients, current clients, the public, other professionals and staff.

 

 

Schedule and complete the nutrition program assessment to evaluate eligibility for congregate and home delivered meal programs.

  • Schedule appointments to complete initial and annual nutrition assessments within the required deadline.
  • Complete in-person assessment(s) for the congregate and home delivered meal program including a nutrition risk screening, evaluation of specials diet needs, access to nutritious meals, and eligibility determination.
  • Assessment may include options counseling to review additional needs of the individual and to provide resources and referrals for in-home and community-based services.
  • Data entry of nutrition assessments and complete necessary documentation in the department Management Information System (MIS) and State MIS system (KAMIS).  

 

Provide Nutrition Education one-on-one or in a group setting at congregate sites.

  • Complete the Nutrition Coach Training and develop materials and presentations to use during one-on-one or group education sessions.
  • Provide nutrition coaching during assessment(s) to address any identified nutrition concerns and provide educational materials when appropriate.
  • Provide scheduled group education at congregate meal sites to educate attendees on useful nutrition facts that will improve their understanding of the importance of healthy food choices and healthy eating.

 

Provide support to the Nutrition Coordinator assisting with day-to-day tasks and providing site management support as needed.

  • Provide part-time support as a congregate site manager at designated meal locations as needed for the daily meal program.
  • Deliver supplies to congregate sites and relay program information from the sites to the Nutrition Coordinator.
  • Develop professional relationships with site managers/senior center directors to support positive long-term partnership and good communication between the congregate sites and CPAAA.

 

Minimum Qualifications: High school diploma or state approved equivalent. One (1) year of experience in an office and/or data entry position; and one (1) year of experience in human service field such as senior services, assessment/interviewing, or a related field. Intermediate computer skills needed in Microsoft Suite. Per Sedgwick County policy, this is a driving level position that requires a valid Kansas driver’s license without restrictions and current proof of automobile insurance. Obtain and maintain Nutrition Coach Training and Certification within six months upon hire.

 

Preferred Qualifications: Associate’s degree from an accredited college or university. One (1) year experience in the field of aging, physical disabilities or nutrition services preferably at an Area Agency on Aging or Physical Disabilities provider system. AIRS, Information and Referral/Assistance Certification. Bilingual in English and a language other than English.
