Responsible for assisting individuals and families through the options counseling process, which includes resource education, assessment, options counseling and referral. Administers functional assessment, and provides back up support for annual functional assessments and intake staff. Responds to requests for information and educates applicants and stakeholders about the I/DD system. Engages and facilitates informed decision-making about accessing long-term services and supports in the community with individuals/guardians.
Service Access
- Assist individuals in accessing the I/DD service system
- Schedule appointments, assessments, and follow-up notifications
- Document information related to contacts and transactions
Education & Referral
- Respond to requests for information
- Educate stakeholders on the available service options and the status of funding
- Educate stakeholders on local and state-wide community resources
- Assemble and distribute resource packets containing affiliate service provider information
- Process targeted case management referrals based on consumer/guardian selection
- Provide documented options counseling for new access, referrals, and provider changes
Assessment
- Complete the Kansas Department for Aging and Disability Services (KDADS) required training prior to performing assessments
- Coordinate and administer annual functional assessments that are completed either via video call or in-person at the choice of the individual/guardian
- Coordinate and administer functional assessments for individuals being offered access to HCBS-IDD Program funding
- Coordinate and administer request for functional assessment from other CDDOs for individuals residing in Sedgwick County
- Participate in all Statewide functional assessment roundtable meetings/trainings
Minimum Qualifications: Bachelor’s degree from an accredited college or university. Education may be substituted with experience, with six (6) months’ experience working with I/DD for each semester. One year of experience as a case manager working with people with an intellectual / developmental disability (I/DD) or any equivalent combination of training and experience that provides the required knowledge, skills, and abilities. Per Sedgwick County policy, this is a driving level position that requires a valid US driver’s license without restrictions and current proof of automobile insurance. Must pass KBI, DCF child abuse, adult abuse registry, and motor vehicle screens.
Preferred Qualifications: Bilingual in English and a language other than English.
