SCA Case Manager III - Aging and Disabilities

Complete assessments and provide case management services to persons 60 years of age and older residing in Sedgwick, Butler and Harvey Counties requesting Senior Care Act (SCA), In-Home services. 

 

Complete assessment and reassessments for clients referred to and/or active on the SCA program.

  • Travel to consumer’s location in the tri-county area of Butler, Harvey, and Sedgwick Counties required.
  • Utilize active listening skills while interviewing clients to collect information and evaluate environment in order to complete assessment tool to determine functional eligibility and make service recommendations.
  • Finalize and review Uniform Assessment Instrument (UAI) to ensure all fields have been accurately completed for data entry, billing, and service authorization.
  • Complete Notice of Action (NOA), Plan of Care (POC), and Customer Service Worksheet (CSW) to initiate and/or continue client services.
  • Accurately complete documentation on all billable activities related to the assessment, in required timeframe.

 

Provide Case Management to ensure clients are receiving appropriate care, services and support throughout the duration on the program.

  • Complete monitoring (via phone call or face to face visit) and follow-up activities every 90 days to assess current health status, examine service provision and address any unmet needs.
  • Implement continual POC revisions to reflect current authorized services, service provider(s), and case closure.
  • Provide accurate and unbiased information and/or referrals as needed; including providing pertinent materials and educating clients regarding resources and care options.

 

Complete client service activities, documentation, and record management in compliance with KDADS policy guidelines.

  • Complete follow-up activities, documentation in the electronic client record Management Information System (MIS), and file management to remain in compliance with policy guidelines.
  • Schedule appointments for assessments and conduct monitoring home visits in an efficient and effective manner.
  • Ensure Microsoft Outlook calendar accurately reflects scheduled activities.
  • Maintain confidential information in accordance with HIPAA guidelines.
  • Contact individuals on SCA waitlist to review program guidelines and determine if the client is interested in pursuing services; and if so, schedule an appointment to complete a UAI.
  • Provide resources information and/or complete appropriate agency referrals if the client is not interested in or suitable for the SCA program.

 

Minimum Qualifications: Associate’s degree in Nursing. Requirement for Associate in Nursing only: Possess and maintain a valid, unencumbered, and unrestricted State of Kansas Registered Nursing License (RN) OR Bachelor’s degree from an accredited college or university in Social Work, Gerontology, Nursing, Counseling, Family Studies, Human Development, Criminal Justice, Psychology, Sociology, or a Health or Community Service related field OR An individual providing Case Management services through an AAA prior to September 30, 2015. One year of work or personal experience in the field of human services such as: working with older adults, working with people with disabilities, mental health, Brain Injury, or closely related field. Per Sedgwick County Policy, this is a driving level position that requires a valid US driver’s license without restrictions and current proof of automobile insurance.

 

Preferred Qualifications: Two years’ experience in the field of aging and disabilities.  Two (2) years’ Case Management experience. Master’s degree from an accredited college or university in Social Work, Gerontology, Nursing, or Counseling. Bilingual in English and a language other than English.
