SCA Project Coordinator - AGING

Manage and oversee the Senior Care Act (SCA) program. Provide program oversight, critical program decision making and staff supervision for the SCA program. Serve as SCA program subject matter expert, provide supervision and training to staff. Work closely with the Kansas Department for Aging and Disability Services (KDADS), Quality Review staff to ensure compliance with state regulations, guidelines and program policies. Engages in ongoing communication with Butler and Harvey County regarding SCA services. Ensure compliance with the SCA contract with the state of Kansas, and provide oversight of the sub-contract with Butler County. Provide hiring recommendations, supervise and evaluate program staff. Conduct assessments, plan of care development, and management of services for persons 60 years of age and older, allowing the consumer to age in place in the community. Monitor the SCA funding process; write and renew contracts; and monitor SCA in-home services.

 

PROGRAM MANAGEMENT

  • Monitor the Wait List and produce timely, accurate reports to all funding and oversight entities.
  • Maintain partnership with Butler County Department on Aging (BU DoA) to provide oversight, coaching, quality review, and direction to their staff who provide contracted services for SCA program in Butler County.
  • Assist with Department contracts and service monitoring.
  • Monitor client usage of services and work with client and provider to resolve concerns.
  • Manage SCA program operations to ensure compliance with contract requirements.
  • Provide quality assurance for the program through continual monitoring to ensure contract with KDADS remains in compliance and quality standards are met. Complete quality review of monthly provider billing, forms and case management activities completed by case managers, and reviews data entry completed by fiscal associate.

 

PERFORMANCE MANAGEMENT

  • Supervise program staff and maintain oversight on the specific program budget, service provided, staff activities, data collection and adherence to funding requirements.
  • Create job descriptions and goals; participate in hiring, evaluation and disciplinary actions, oversee caseloads and quality of work, manage staff schedules and serve as backup to the Unit Director as needed.

 

CASE MANAGEMENT & ASSESSMENT

  • Complete the SCA Uniform Assessment Instrument (UAI) to determine eligibility and appropriate care choices for SCA In-Home program.
  • Develop Plan of Care (POC) and perform ongoing management of services to individuals receiving SCA services.
  • Prepare and maintain client files in accordance with HIPAA and program guidelines.
  • Provide case management to ensure clients are receiving appropriate care, services, and support throughout the duration of their time on the program.

 

Minimum Qualifications: Bachelor’s degree from an accredited college or university with a major in Social Work, Gerontology, Healthcare Administration or closely related human service field. Minimum of two to two (2) years’ experience in the field of aging or physical disabilities and case management experience with an emphasis in administration and/or project management. Supervisory experience. Valid State of Kansas driver’s license and proof of automobile insurance.

 

Preferred Qualifications: Master’s degree from an accredited college or university in Social Work, Gerontology, Healthcare Administration or closely related human service field. Three (3) years’ or greater of work experience program coordination / project management; experience writing contract agreements and grants; or any equivalent combination of experience which provides the required knowledge, skills, and abilities. Licensed Social Worker. AIRS, Information and Referral Certification. Bilingual skills a plus.
