The Access and Discharge Team Clinical Support provides clinical expertise, consultation, and support to the Certified Community Behavioral Health Clinic’s (CCBHC) Access and Discharge Team. This position exists to ensure clinically sound, evidence-based, and person-centered care for youth and families transitioning between levels of care. While the employee may provide direct therapeutic services as needed, the primary focus of this position is to enhance clinical quality, facilitate case review, and strengthen team capacity to maintain youth in their communities.

 

The Access and Discharge Team Clinical Support will participate in CBST/PRTF meetings, provide clinical recommendations regarding treatment modalities and interventions, and collaborate with internal and external providers to promote continuity and appropriateness of care. The employee may provide community education and training related to alternatives to congregate care, emphasizing the clinical and developmental impacts of residential treatment.

 

Clinical Consultation and Support

  • Provides clinical consultation, collaboration, and as-needed direct services to support evidence-based, trauma-informed, person-centered care, ensuring clinically sound treatment planning, coordinated transitions, and safe community-based alternatives for youth with complex needs.
  • Community Education and System Support
  • Delivers education, consultation, and liaison support to community partners and stakeholders to promote shared understanding, collaboration, and the use of strengths-based, least-restrictive, community-based treatment approaches.
  • Quality and Best Practices
  • Maintains expertise in evidence-based children’s mental health practices, supports program evaluation and CCBHC standards, and ensures care coordination, discharge processes, and documentation align with best-practice clinical guidance.

 

 

Minimum Qualifications: One (1) year of experience providing therapeutic services to youth and/or adults. Master’s Degree or higher. Must be licensed by the Behavioral Sciences Regulatory Board as an LMSW, LMFT, LMLP, LPC, or LP. Employees hired with a temporary license must test for permanent licensure within 90 days of hire. Per Sedgwick County policy, this is a driving level position that requires a valid US driver’s license without restrictions and current proof of automobile insurance. Must have access to a personal passenger vehicle to complete the driving duties of the position. Meet the specifications as outlined in the CMHC, CCBHC, and CIC licensing standards and pass KBI, DCF child abuse check, DCF adult abuse registry, KDADS Nurse Aide and Criminal Record check, National Sex Offender Registry check, and motor vehicle screens. Must complete orientations provided by Sedgwick County and COMCARE.

 

Preferred Qualifications: One (1) year of experience participating in or facilitating CBST and/or PRTF meetings. Knowledge of state and federal regulations governing residential and community-based treatment services. Experience developing or delivering community-based training or education on mental health topics. Familiarity with Medicaid and HCBS SED Waiver programs. Demonstrated ability to provide clinical guidance and consultation to multidisciplinary teams. Experience with program development, quality improvement, or implementation of evidence-based practices within a behavioral health setting. May be clinically licensed by the Behavioral Sciences Regulatory Board as an LCP or LCPC, LCMFT, or LSCSW.
