Administrative Case Management (ACM) ensures individuals who strive to remain in their community with in-home services are able to seamlessly access services through a streamlined eligibility process. Administrative Case Management shall be available to Sedgwick, Harvey, and Butler County consumers determined functionally eligible for Home and Community Based Services (HCBS) Physical Disability (PD) Waiver, Frail Elderly (FE) Waiver, Brain Injury (BI) Waiver, or the Program of All-Inclusive Care for the Elderly (PACE). ACM includes Medicaid Application Assistance and Liaison services.

 

Facilitate the financial application process for clients applying for Medicaid

  • Assist Home and Community-Based Services (HCBS) waiver consumers in gathering and completing paperwork for Medicaid financial initial determination and Medicaid recertification.
  • Initiate and coordinate meetings with HCBS waiver consumers; act as a single point of contact for consumers during the Medicaid process.
  • Follow up with Medicaid financial eligibility entity to ensure application was received.
  • Monitor and update HCBS waiver consumers on application status until application determination.

 

Assist consumers to obtain supporting documentation for HCBS waivers

  • Assist with obtaining documentation to support initial and recertification program eligibility for Medicaid
  •  Assist with the completion of the Consumer Evaluation of Needs form for consumers requesting Crisis Exception.
  • Assist with obtaining supporting documentation for Crisis Exception Requests for applicable HCBS waiver.

 

Administrative functions

  • Complete required documentation and billing in required time frame of the service contract.
  • Maintain consumer files in accordance with ACM and HIPAA guidelines.
  • Complete customer satisfaction survey and data enter survey results.
  • Provide quality assurance data for the program.
  • Ensure required Medicaid and Department mandated trainings are completed.

 

Minimum Qualifications: Associate’s degree from an accredited college or university in Social Sciences, Humanities, Public Health, or relevant field of study. Degree can be substituted by two (2) years of relevant experience and high school diploma or state approved equivalent.  Minimum one (1) year of experience working with the aging and disability populations or 1 year of experience working with the Kansas Medicaid population.  Basic computer skills. Per Sedgwick County policy, this is a driving level position that requires a valid Kansas driver’s license without restrictions and current proof of automobile insurance.

 

Preferred Qualifications: Two (2) years’ experience in the field of aging and disability. Bachelor’s degree from an accredited college or university in Social Sciences, Humanities, Public Health, or relevant field of study. Bilingual in English and a language other than English.
