This is a professional staff position that provides direct services to older adults to promote diabetes education, and health and wellness education. The position will provide diabetes education, wellness education for adults 60+. The position is responsible for assessing the client’s program eligibility for Diabetes Prevention Program (DPP) and Medicare Diabetes Prevention Program (MDPP) through the established program guidelines. The position will lead educational programming within the agency, partners and community members to promote 60+ health and wellness programming. This position will offer programming to promote health promotion and disease prevention, physical activity, a healthy lifestyle and positive aging.
PROGRAM MANAGEMENT
- Adhere to funding/program requirements and ensure all grant requirements are met
PROVIDES EDUCATION
- Conduct classes or presentations promote health promotion and disease prevention, physical activity, a healthy lifestyle and positive aging
- Lead exercise classes or demonstrate low impact exercise and stretching.
- Provide direct education services to promote diabetes prevention through the DPP or MDPP program for interested clients or employer groups that meet eligibility criteria and maintain the evidence based requirements to ensure the agency fully meets CDC standards
- Promote health and wellness in the community by attending identified meetings, participating in fairs, serving on committees or partnering on special projects
- Provide direct education services to community groups to promote health promotion and disease prevention and increase referrals to the three Diabetes programs.
PERFORMANCE MANAGEMENT
- Schedule classes and appointments to meet requirements
- Monitor the program(s)’ quality improvement to meet requirements and enhance programming/outreach efforts.
- Ensure necessary data (client data, case notes) is collected and data entered as appropriate to meet current requirements.
- Work with support staff and billing staff to ensure that all necessary paperwork and documentation is accurate and meets Medicare billing standards.
Minimum Qualifications: Bachelor’s Degree in Exercise Science, Health Science, nursing or health-related field from an accredited college or university. Minimum of one (1) year experience in the field of health and wellness, community education, diabetes education, working with older adults or a combination. Will be expected to obtain coaches training for the Diabetes Prevention Program within first two weeks of employment.
Preferred Qualifications: Master’s Degree in Exercise Science, Health Science, nursing or health-related field from an accredited college or university and coaches certification for the CDC’s Diabetes Prevention Program. Ideal candidate will have: three (3) years’ or greater of work experience in the aging field for an Area Agency on Aging with knowledge of diabetes education, health and wellness programming, experience with community education and knowledge of CDC Diabetes Prevention Programs.
