Provides leadership and oversight of the day-to-day functions of the CCBHC Care Coordination program.  Focus is on population health and integrated care management activities designed to treat the “whole person” by encouraging health behaviors and lifestyle.  

 

Personnel Management

  • Defines core competencies for all program positions and establish a protocol for assessing hard and soft skills through routine observation of work with patients and team members.
  • Coaches, mentors and motivates project team members, ensuring their individual accountability for assigned work.
  • Provides leadership, training, and coaching to staff, including the discussion, addressing and documentation of personnel issues in a timely manner.
  • Conducts regularly scheduled supervision sessions with direct reports and team meetings with care coordination team.
  • Oversees new staff orientation and ongoing staff training and assures that all staff have completed required state and other mandatory training.
  • Ensures compliance with regulations and policies for billing, documentation and protected health information.
  • Promotes a culture of customer service excellence and continuous quality improvement through strategic team planning and problem solving.

 

Program Planning, Oversight and Implementation

  • Develops and monitors short and long range planning initiatives relative to population health management and care coordination activities with a focus on continuous improvement and meeting CCBHC requirements related to care coordination.
  • Ensures employees are providing timely outreach, services and documentation of deliverables in accordance with performance goals established for the program.
  • Ensures input from patient, care team, or other persons who provide support and service to our patients.

 

Population Health Management and Cross System Collaboration

  • Establish tracking mechanisms for outcome measures defined in the CCBHC Criteria.
  • Produce program reports summarizing key goals, accomplishments and patient outcomes.
  • Stay abreast of new developments and industry trends related to care coordination, population health management, clinical quality improvement and best practices.
  • Develop resource lists to identify specialty services, provider groups, social support organizations, and other community resources.

 

Minimum Qualifications: Three years of progressively responsible experience in administrative and leadership roles. One year of supervisory experience or equivalent leadership experience. Two years of experience managing relationships across different teams, departments, or external partners. Bachelor’s degree from an accredited college or university in a social service field or nursing. Per Sedgwick County policy, this is a driving level position that requires a valid US driver’s license without restrictions and current proof of automobile insurance. Must have access to personal passenger vehicle to complete the driving duties of the position. Must be able to safely operate a motor vehicle and meet driver qualification standards as set forth in Sedgwick County’s Fleet Vehicle Operation and Usage policy.   Meet the specifications as outlined in the CMHC/CCBHC licensing standards and pass KBI, DCF child abuse check, DCF adult abuse registry, KDADS Criminal Record check, National Sex Offender Registry check, and motor vehicle screens.  Must complete orientations provided by Sedgwick County and COMCARE.

 

Preferred Qualifications: Master’s degree from an accredited college or university in a social service field or nursing. Knowledge of federal, state, local and contract regulations and requirements including performance outcomes. Three years’ relevant work experience with a CMHC/CCBHC. Two (2) years supervisory experience. Licensure by BSRB at Master’s Level.
