Pay:  $25.87 per hour | This position may be eligible for a sign on bonus of $2,500 after 60 days of employment if selected applicant meets all policy conditions | This position may be eligible for shift pay of $3.00 per hour for hours worked between 6:00 pm and 6:00 am.  

Work Schedule: Monday - Friday between 7:00am-6:00pm, varied schedule

Collaborate with both inpatient and outpatient treatment team to ensure the patient residing at Osawatomie State Hospital (OSH) receives the best possible inpatient and outpatient psychiatric treatment.  Plan and coordinate discharge of Sedgwick County residents from the inpatient setting at OSH back to the community.

 

Monitor Inpatient Census at State Hospital.

  • Communicate and collaborate with outpatient treatment team to collect and retrieve information regarding past patient services. 
  • Prepare weekly census report inclusive of all admissions, transfers, discharges and relative information and fax to designated COMCARE, OSH and community locations.
  • Confer with both inpatient and outpatient medical and treatment team to exchange information regarding care and treatment and potential barriers or problems.
  • Read medical record and complete face-to-face interviews to assess and offer input on patient cognitive organization, affect and mood.
  • Research available services, terminated services, denial of services, or any other facet that is preventing appropriate placement, discharge, and follow-up care.

 

Facilitate Discharges from Inpatient Setting.

  • Develop discharge plans for patients in the state hospital. 
  • Set appropriate appointments for medical follow-up for patients with medical issues.
  • Assist integrated care specialists and/or medical team after discharge to provide information as requested, clarification of medication orders, assist with questions, problems, or facilitation of services by providing guidance, feedback, and community resources.
  • Prepare written discharge plans to COMCARE staff, affiliate agencies and the state hospital prior to a patient being discharged.
  • Monitor the implementation of discharge plans to ensure the patients have all recommended services promptly on their discharge from the state hospital.
  • Act as a liaison between all the COMCARE programs and affiliates regarding consumers that are inpatient at the state hospital. 
  • Work to remove service barriers to enhance service delivery upon discharge from inpatient setting to prevent subsequent hospitalization.

 

Primary CSS Point of Contact for Court Issues.

  • Work with primary COMCARE court liaison housed at Crisis to serve as point of contact for CSS and the courts in regards to mental health petitions, Ex Parte, outpatient treatment orders and court reports.
  • Establish tracking form and develop record-keeping system of all court documents. 
  • Collaborate with the state hospital liaison to establish outpatient treatment, order records, and input appropriate information in to Psych Consult system.
  • Communicate with Inpatient staff at VCGS, Osawatomie State Hospital and Larned State Hospital to ensure accurate reporting of CSS court related information.

 

Minimum Qualifications: Bachelor’s degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education. Per Sedgwick County policy, this is a driving level position that requires a valid US driver’s license without restrictions and current proof of automobile insurance. Must have access to personal passenger vehicle to complete the driving duties of the position. Must be able to safely operate a motor vehicle and meet driver qualification standards as set forth in Sedgwick County’s Fleet Vehicle Operation and Usage policy.  Meet the specifications as outlined in the CMHC/CCBHC licensing standards and pass KBI, DCF child abuse check, DCF adult abuse registry, KDADS Nurse Aide and Criminal Record Check, National Sex Offender Registry Check, and motor vehicle screens.  Must complete orientations provided by Sedgwick County and COMCARE.
