Collaborate with both inpatient and outpatient treatment teams to ensure patients residing at State Institutional Alternative (SIA) hospitals and other diversion alternatives receive the best possible inpatient and outpatient psychiatric treatment. Plan and coordinate discharge of Sedgwick County residents from the identified setting back to the community.
Employee must be able to complete complex tasks by applying CCBHC practices standard office policies, authorized instructions, and past precedents to achieve a desired outcome. Employee must quickly recognize emergency or sensitive situations and take appropriate action. Critical decision making and the ability to think through the consequences of a decision are essential in this position.
Monitor Inpatient Census at SIA Hospital and other diversion placements.
- Communicate and collaborate with outpatient treatment team to collect and retrieve information regarding past patient services. Obtain history and recommendations from treatment providers, family members, friends, and other agencies as appropriate to assist in developing the discharge plan.
- Solicit and provide information to/from patient regarding services, support systems, potential problems, medical needs, follow-up appointments, transportation, etc.
- Read medical records and complete face-to-face interviews to assess and offer input on the patient’s functional ability to participate in services.
Facilitate Discharges from Inpatient Setting.
- Develop discharge plans for patients in the SIA hospitals and diversion alternatives. These plans will include, but not be limited to housing status, medication management appointments, financial resources, psychosocial day programs, case management services, home health services, medical appointments, substance abuse treatment services, etc.
- Act as a liaison between all the COMCARE programs and affiliates regarding patients that are inpatient at the SIA hospital. Provide feedback and direction to SIA team regarding the determination of services needed, availability, barriers, and solutions.
- Provide transportation to support client attendance in medical appointments and other therapeutic goal-related activities to include development of community resources, employment or education access, social or consumer-run activities and related supports
Documentation
- Complete progress notes in accordance with COMCARE and CCBHC guidelines which meet requirements for medical necessity, goal-directed treatment and patient response to treatment within set timelines
- Complete Notes for Record, releases of information, safety documents and other documentation as required and/or recommended within established deadlines
Minimum Qualifications: Bachelor’s degree in a human services field or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education. Per Sedgwick County policy, this is a driving level position that requires a valid US driver’s license without restrictions and current proof of automobile insurance. Must have access to personal passenger vehicle to complete the driving duties of the position. Must be able to safely operate a motor vehicle and meet driver qualification standards as set forth in Sedgwick County’s Fleet Vehicle Operation and Usage policy. Meet the specifications as outlined in the CMHC/CCBHC licensing standards and pass KBI, DCF child abuse check, DCF adult abuse registry, KDADS Nurse Aide and Criminal Record check, National Sex Offender Registry check, and motor vehicle screens. Must complete orientations provided by Sedgwick County and COMCARE.
